Please feel free to submit this completed form to us regarding your dig locate request. Please note that we will not accept any request unless this form is filled out completely. You must also agree to the following disclaimer before we will process your request. If you do not click on the "I Agree to Above Disclaimer" below, we will not be able to process the locate. After we receive the request, we will process it within the time frame listed below and send you a confirmation via your choice, either by return E-Mail or by fax.

DISCLAIMER: I understand that Mississippi One-Call System has until noon (12:00pm) on the next business day to enter this request(s) provided Mississippi One-Call System receives it by 1:00pm today (Monday - Friday excluding holidays). If MOCS receives this request after 1:00pm today (Monday - Friday excluding holidays), they will have until 3:00pm on the next business day to enter the ticket. I agree to and understand that my 2 working day waiting period will not begin until the above mentioned times. I also agree that I may only request routine tickets (2 working day notice - excluding weekends & holidays).
